Since the early times of rhinoplasty, surgeons have been busy with shaping the bony and cartilage tissues mostly. However, nowadays, the importance of soft tissues in addition to the bone and cartilage has been demonstrated.
One of the most important elements of the soft tissue system of the nose is the Pitanguy ligament system. It is important in the movements and position of the nasal tip.
This ligament is damaged in open rhinoplasty but is protected in closed rhinoplasty. Protection of this ligament has positive effects on the aesthetic and dynamic outcomes.
Since the Pitanguy ligament and the surrounding soft tissues are protected, it is possible to avoid the problem of a frozen nasal tip, which is the situation in many of the patients following open rhinoplasty. We achieve a dynamic and motile nasal tip with the help of the closed technique.

Why Closed Rhinoplasty?
Here is some basic information about the nasal anatomy to understand the philosophy of closed rhinoplasty: The main structure of the nose is composed of bone, cartilage, and the soft tissues under the skin.
Since the early times of rhinoplasty, surgeons have been busy with shaping the bony and cartilage tissues mostly. However, nowadays, the importance of soft tissues in addition to the bone and cartilage has been demonstrated.
One of the most important elements of the soft tissue system of the nose is the Pitanguy ligament system. This ligament system progresses under the skin along the back of the nose, passes the tip and holds onto the nasal root at the lips. It is an elastic ligament and is important in the movements and position of the nasal tip. It is in close relationship with the muscle that pulls the nasal tip and the lip downwards.
This ligament is damaged in open rhinoplasty but is protected in closed rhinoplasty. Protection of this ligament has positive effects on the aesthetic and dynamic outcomes.
Rhinoplasty Without Nasal Tampons
In the final step of rhinoplasty operations, tampons are placed within the nostrils in order to reduce the post-operative oedema and to provide intranasal support. The tampons used in the past were made up of sterile cottons. They used to adhere to the recovering tissues within the nose; thus, it was painful and caused bleeding during removal. Removal of the tampons caused anxiety for the patients more than the operation itself.
Currently, silicone support materials are used instead of the cotton plugs. Since they do not adhere to the intranasal tissues, removal of silicone supports is easy and painless. Another advantage of silicone supports is that they have air tunnels inside. Thus, the patient can efficiently use the nasal sprays and if these tunnels are not obstructed with clot, the patient can easily breath until its removal.
Nasal Awareness
Patients should be aware of the nasal defects they have before they opt for nasal aesthetics. It is important that the patients describe the problems that cause discomfort. The first question to be directed to the patients should be: which part of your nose are you uncomfortable with? The answers may be one of these: ‘’My nose is a Roman nose”, ‘’the tip part is low’’, “my nostrils are wide”, ‘’my nose is deviated’’, ‘’my nose is different when viewed from the right and the left’’, or ‘’my nose tip is large’’
It is easier to progress with patients who are aware of their nasal defects and who can describe what they want.
Revision (Secondary) Rhinoplasty
Revision operations are special cases that exceed a classical rhinoplasty. It is extremely important to understand the underlying anatomical problem. Another importance of inner nasal examination in revision rhinoplasty is to understand the presence of a sufficient cartilage stock. If the septum cartilage is mostly preserved in the first operation, the amount of the cartilage would be sufficient for the revision operation. However, if the amount of the cartilage is not sufficient, cartilage transfer from another region may be necessary.
There are two different sources of the body that provide cartilage for transfer: the ear and the rib. If a small and soft piece is needed, ear cartilage would be sufficient. If a larger, steady, and plain piece is needed, a rib could be used.
Let Down Technique
There are some basic differences between classical rhinoplasty and the “let down” technique. In classical rhinoplasty, the nasal arch is corrected by direct removal of the cartilages or bone. The archy appearance is corrected; however, the natural appearance of the back of the nose is lost. Following classical rhinoplasty, patients feel irregularities when they touch their nose; it is not a smooth nose anymore.
In “let down” rhinoplasty, correction of the archy part in provided, not by direct removal of the cartilages or bones, but by removal of the deeper structures. Therefore, the nasal dorsum lights are not deteriorated, and patients do not feel irregularities when they touch their nose.
The “Let down” technique necessitates surgical experience.
MICROSAW RHINOPLASTY
The most important step of rhinoplasty is construction of the bony structures.
In conventional rhinoplasty, devices such as rasp, hammer and chisel are used, which are rough and have the potential to harm the bone. With the rapid development of modern rhinoplasty, the devices used have changed and developed. Micro-saws are one of these devices. Using rasp, hammer or chisel have led to common unwanted outcomes such as fractures, surface irregularities, excessive post-operative bleeding and palpable irregularities in the nose.
The bone can be shaped smoothly using the devices called micro-saws that have millimetric tips. The devices cause minimal harm to the bone and the soft tissues under the bone; thus, the post-operative bruising and swelling become less frequent.
Profiloplasty (Profile Aesthetics)
Profile aesthetics deals with the side-appearance of the face. The nose is the most prominent organ from a profile view; however, when the face is evaluated as a whole, the chin tip, the region between the two eyebrows, the forehead, the eyebrows, the cheekbones, the eyelids, and the lips are also important. Profile aesthetics aims to correct the compliance between the mentioned elements, making the side-appearance of the face aesthetic.
Despite having acquired a beautiful nose following rhinoplasty, if the tip of your chin is further back than normal, your facial compliance will be incomplete.
